In essence, Nightmare Dungeons only appear in World Tier Three: Nightmare difficulty and above, but they are not instantly available once players move up the World Tier. Nightmare Dungeons can only be accessed and activated once players have found the corresponding Nightmare Sigil, which are random drops from enemies out in the world.

Each sigil only points towards a specific dungeon, so there can be a variety of different sigils of differing quality. Each sigil also comes with different dungeon affixes that make the Nightmare Dungeon what it is. Not only do players have to contend with stronger and more dangerous foes, but these modifiers also oftentimes warrant a change in strategy.

For example, players may get stronger Frost Damage, but at the expense of a recurring lightning storm that hits everything outside a barrier of safety, while monsters get increased attack speed.

Finishing a Nightmare Dungeon allows players to interact with an Awakened Glyphstone, which gives several points to help level up Glyphs that are used in the Paragon Board system.
